TOWN, COUNTY, MATERIAL COVERING THE AGENCIES HISTORICAL BACKGROUND (1ST TIME APPLICANTS), AND COPY OF 501(C)3, COPY OF CURRENT 990 FORM, COPY OF CURRENT FINANCIAL AUDIT (PURSUANT TO NYS NONPROFIT REVITALIZATION ACT OF 2013 GUIDELINES), LIST OF BOARD OF DIRECTORS INCLUDING BUSINESS AFFILIATIONS, LIST OF CURRENT BUSINESS CONTRIBUTORS AND FOUNDATION GRANTS INCLUDING THE LEVEL OF SUPPORT.
