THE FOUNDATION'S TREASURER AND TAX PREPARER WAS UNDERGOING SIGNIFICANT MEDICAL TREATMENTS AT THE TIME AND HAS SUBSEQUENTLY PASSED AWAY. HE DID NOT MAKE THE FOUNDATION AWARE OF THE NEW REQUIREMENT TO E-FILE THE RETURN. THE FOUNDATION HAS HISTORICALLY FILED ITS RETURNS AND PAID ALL TAXES DUE ON A TIMELY BASIS. THE FOUNDATION HAS SINCE HIRED A NEW ACCOUNTING FIRM TO PREPARE AND FILE ITS RETURNS. WE ASK THAT ANY PENALTIES FOR LATE FILING BE WAIVED.
