I am mortified and apologize for the late filing and take full responsibility. This foundation was formed after my sister Maizie died of lung cancer in June 2001 at the age of 52. She wanted to support lung cancer research so we formed it soon after her death with a fiscal year ending June 30. I prepare the return myself and have found the fiscal year and resulting filing dates confusing since they don't correspond with any other filings I have to make. I rely on my assistant to remind me as he keeps the timeline of all returns and other filings due. However he has been busy and also has had personal issues to deal with. Again I apologize and regret very much that this happened. I will take steps to make sure It won't happen again.
