How to Apply Application should be submitted on the Kubly Family Foundation Scholarship Loan Program for Higher Education Application which can be found at www.kublyfamilyfoundation.org. The following materials must be included with the application:- Proof of high school graduation or GED- Verification of undergraduate degree and GPA- Verification of current graduate school GPA, if applicable.- Verification of graduate/professional degree program acceptance- Current transcript with verification of enrollment status (i.e., junior status and entering third year)cumulative GPA, and declared major for undergraduate degree programs- Copy of tuition bill for the first scholastic period. This may be furnished upon receipt by the scholarship recipient.- Completed application form and essay questions.- Three references that the Committee may contact. Only one reference may be a direct relative.If requested, conduct an interview with the Selection & Oversight Committee.
Restrictions The applicant must have a current acceptance into an accredited school offering bachelors, graduate,or professional degree level programs. For graduate/professional level degree program, applicants must have an undergraduate degree with a cumulative grade point average of 3.0 or greater on a 4.0 scale. For undergraduate degree programs, applicants must be at least junior status, entering their third year of undergraduate studies, and officially have declared a major. Current undergraduate degree programs eligible for application to the Scholarship Loan Program are early childhood education, primary education, secondary education, and Spanish. Applicant must demonstrate leadership qualities, show a committment to return to Monroe, Wisconsin area and have a desire to transform Monroe into a better place to live and work.
