The Foundation provides grants to organizations which are exempt from taxation under Section 501(c)(3). An organization seeking a grant from the Foundation is required to submit an online application and provide the following information: . Name, address, phone number and contact person of the organization . Mission of the organization . Description of the project for which grant funds are being requested . Names of the organization's board of directors . Verification of the organization's tax-exempt status . A current financial statement . Cost of the proposed project and the amount being sought from the Foundation . Budget for the proposed project . Organization's budget . A list of other sources of support and the amounts and types of their donations . An explanation of whether this is an ongoing activity or a one- time project
