SCHOLARSHIP APPLICANTS MUST PROVIDE A CURRENT APPLICATION FORM FROM HTTPS://WWW.DVMC.COM/ABOUT-US/FOUNDATION.ASPX, AT LEAST TWO LETTERS OF REFERENCE FROM SCHOOL OR WORK, OFFICIAL HIGH SCHOOL/COLLEGE TRANSCRIPT AND SUPPORTING DATA, PERSONAL STATEMENT OF PLANS AND GOALS, AND A COPY OF THE MOST CURRENT PERSONAL INCOME TAX RETURN.
