GRANT APPLICATION FORM ACCOMPANIED BY THE FOLLOWING SUPPORTING DOCUMENTS: 1) DATED COVER LETTER SIGNED BY BOARD PRESIDENT OR EXECUTIVE DIRECTOR STATING THE PURPOSE, AMOUNT OF REQUEST, AND BOARD ENDORSEMENT OF REQUEST. 2) OVERVIEW OF ORGANIZATION'S CURRENT PROGRAMS, ACTIVITIES, AND SERVICES. 3) SUMMARY OF THE PROJECT WHICH INCLUDES NEED STATEMENT, PROGRAM GOALS/OBJECTIVES, AND IMPLEMENTATION STRATEGIES. 4) PROPOSED BUDGET FOR THE PROJECT, INCLUDING LIST OF OTHER FUNDINGS SOURCES CONTACTED FOR THE PROJECT. 5) FUTURE FUNDING PLAN FOR CONTINUING THE PROJECT.
Restrictions
GRANTS ARE AWARDED TO QUALIFIED CHARITABLE ORGANIZATIONS WHO SERVE THE RESIDENTS OF TUSCARAWAS COUNTY. THE FOUNDATIONS CHARITABLE GRANT MAKING INCLUDES BOTH CAPITAL AND PROGRAM GRANTS WHICH ARE AWARDED IN THE FOLLOWING CATEGORIES: ARTS, CULTURE, & HUMANITIES; CIVIC & COMMUNITY AFFAIRS; CONSERVATION & ENVIRONMENT; EDUCATION; HEALTH & WELLNESS; AND HUMAN SERVICES. GRANTS ARE LIMITED TO ACTIVITIES PERFORMED AND ORGANIZATIONS LOCATED WITHIN THE UNITED STATES. GRANTS MAY NOT BE USED TO CARRY ON PROPAGANDA OR OTHERWISE ATTEMPT TO INFLUENCE LEGISLATION, PARTICIPATE IN OR INTERVENE IN ANY POLITICAL CAMPAIGN ON BEHALF OF OR IN OPPOSITION TO ANY CANDIDATE FOR PUBLIC OFFICE, OR TO CONDUCT, DIRECTLY OR INDIRECTLY, ANY VOTER REGISTRATION DRIVE (WITHIN THE MEANING OF SECTION 4945(D)(2) OF THE INTERNAL REVENUE CODE).
