Submit a written request on your organization's letterhead with printed name of the organization's address, phone number, email, and contact name. Submit no more than a 2-page write-up on the reason for your request, amount requested, and the total amount of the project, purpose of the project, current funding status and other funding sources, outcome expected and how future funding will be accomplished. Include your organization's mission statement and recent campaign/project efforts to include a measurement of success, a list of the organization's current board memebers. Also include a copy of the organization's current 501(c)(3) IRS designation letter and statement confirming your organization's status is current.
