Applications should be mailed to the address above or sent via email to ACFgiving@ameritas.com and include the following information: (a) Organization name, address, EIN/TIN and website; (b) Chief Executive Officer name, telephone number and email address; (c) Contact person name and title, telephone number and email address; (d) Organization Mission Statement and Purpose of Request; (e) Funding request - amount requested, who and how many will be served, partnerships, and work plan; (f) Financial plan - project budget, development plan, and timing; (g) Supporting Material - operational budget, financial statements, review/audit opinion of financial statements, board of directors and staff.
Restrictions
Preference will be given to funding requests from qualified applicants received from communities with an Ameritas presence. All organizations having 501(c)(3) tax-exempt status conferred by the Internal Revenue Service are eligible to apply for funding consideration as long as the organization has two years of audited or reviewed financial statements. Preference will be given to educational programs, but consideration will be given to civic, cultural, health and welfare requests. Generally, requests for support of debt retirement or operational deficits are not favored. Funding of any project, program or organization is limited to 10% of the total amount of the project, program or organizational budget. Funding is also limited to 5% of the total amount for capital campaigns.
