ADVOCARE FOUNDATION GRANT APPLICATION, BRIEF DESCRIPTION OF THE ORGANIZATION'S MISSION, STATEMENT OF NEED, NUMBER AND DESCRIPTION OF PEOPLE WHO WILL BENEFIT FROM THIS PROGRAM, TIMELINE FOR PROJECT START-UP, COMPLETION AND EXPENDITURE OF FUNDS, DESCRIPTION OF DESIRED IMPACT FROM THE PROGRAM AND WHAT EVALUATION COMPONENT(S) WILL BE USED TO MEASURE SUCCESS, ENTITIES ASKED TO GIVE SIGNIFICANT FINANCIAL SUPPORT TO THE PROPOSED PROGRAM, ORGANIZAITONAL BUDGET, PROJECT BUDGET, MOST RECENT AUDITED FINANCIAL STATEMENT, LIST OF BOARD OF DIRECTORS AND COPY OF TAX STATUS LETTER.

GRANT REQUESTS FOR ENDOWMENT FUNDS WILL NOT BE CONSIDERED. GRANT AMOUNTS RANGE FROM $10,000 TO $250,000 (MULTI-YEAR PAYMENT). GRANTS WILL BE AWARDED IN MID-JULY.
