THIS FORM INCLUDES INFORMATION AS TO THE SCHOOL THEY WILL ATTEND, GRADE OR YEAR, ANNUAL TUITION, ROOM & BOARD COST, SCHOLARSHIPS RECEIVED, AMOUNT FUNDED BY PARENTS AND LOANS. THE APPLICATION ASKS FOR INFORMATION CONCERNING THE FAMILIES FINANCIAL SITUATION, TAX FORM 1040 FROM MOST RECENT YEAR, SCHOOL TRANSCRIPTS, SAT SCORES, AND LETTER OF RECOMMENDATIONS.
