How to Apply
The Evelyn Horn Scholarship Application must be completed in its entirety. Applicants may attach copies of any pertinent information they feel with be helpful to the review committee. Applications must be signed by a school official before being mailed out.
Restrictions
The scholarships to the high school students are limited to four years. Once awarded, no scholarship can be terminated unless the Student obtains less than a C average or unless the student fails to carry an average course load on a full time basis.
