a. One page typed essay. b. Application for scholarship. c. High School transcript plus Grade Point Average and class rank must include grades for classes completed through December of current year. d. Official college transcript from all colleges or universities attended grades through December of current year must be included. e. Attach a copy of SAT or ACT scores and rank. f. Letter of Recommendation form attached: 1 Spouses and Children - letter may be obtained from school principal teacher counselor civic leader clergy or employer. 2 Employees - letter is to be obtained from the employee's location or department manager.
Restrictions
Grants will be limited to full-time and part-time undergraduate study in a recognized accredited institution of higher learning i.e. college or university. Excluded are elementary secondary graduate correspondence or any type of education facility other than those specifically included in the limitation stated above.
