See attached Statement Regarding Activity - Part VI A Line 5 which includes a description of termination and liquidation of the assets of the private foundation including the transfer of its assets to Fidelity Charitable 501c3 public charity and copies of the following dissolution documents: the California Secretary of State DISS NP non-profit certificate of dissolution for the Foundation was signed on November 8 2024 by the foundation Directors; the California Department of Justice provided a Dissolution Waiver - with Assets to the Foundation on February 25 2025 granting permission to complete the liquidation of the Foundation; and the document showing that the dissolution documents and the Dissolution Waiver from the California Department of Justice were submitted March 20 2025 to the California Secretary of State.
