APPLICATION FOR GRANTS AND SUPPORT SERVICES IS MADE ON AN APPLICATION PROVIDED TO THE APPLICANT BY THE FOUNDATION. SUPPLEMENTARY MATERIALS TO BE SUBMITTED INCLUDE TAX RETURNS AND MEDICAL RECORDS TO SUPPORT QUALIFICATION.
Restrictions
QUALIFIED APPLICANTS WILL RANGE IN AGE FROM 18 TO 65 AND ARE PATIENTS CURRENTLY RECEIVING OR WILL BE RECEIVING WITHIN 3 MONTHS CANCER TREATMENT. INDIVIDUALS MUST RESIDE IN THE SAN FRANCISCO BAY AREA AND RECEIVE TREATMENT IN THE SANTA CLARA COUNTY. THE PATIENT'S FAMILY MUST HAVE 1 OR MORE DEPENDENTS UNDER THE AGE OF 18 WHO ARE IMMEDIATE FAMILY MEMBERS. PATIENT MUST BE A CITIZEN OF THE UNITED STATES.
