STRONGER PATHWAYS GRANT APPLICATION INCLUDES, NAME, CONTACT, AMOUNT REQUESTED, DESCRIPTIONS OF THE SERVICES THE ORGANIZATION PROVIDES AND EXAMPLES OF HOW THOSE SERVICES HAVE BEEN IMPLEMENTED IN THE PAST. DESCRIBE HOW THE ORGANIZATION WOULD UTILIZE FUNDS AND WHAT ACTIVITES AND OUTCOMES THE ORGANIZATION HOPES TO ACHEIVE AS A RESULT OF THE FUNDS. DESCRIBE IF FUNDING WILL GO TO SUPPORT EXISTING SERVICES OR AN EXPANSION OF SERVICES. ALSO INCLUDE THE DETAILS OF THE GEOGRAPHIC LOCATION WHERE SERVICES OR ACTIVITES WOULD TAKE PLACE. MUST PROVIDE INFORMATION ON HOW THE ORGANIZATION WILL MONITOR THE ACTIVITES AND WHAT TYPE OF REPORTING WILL BE CONDUCTED.
